Sample Urgent Message with Relay Documentation:
The sample message in the image below shows the detailed documentation we
make whenever we have to relay an urgent message. Every single call we make
while attempting to relay is documented with the time and date, the operator's
initials, the person called, and what number we called them on.

The documentation starts below the message. Each line of the documentation
starts with two dashes (i.e., "--") in order to differentiate it from
the actual message. Each line represents a discrete step in relaying your urgent
message.
Once you know how to read our notation, you can see exactly what we
have done on your behalf. For instance, here is what happened with this message:
The first line indicates that we called Bob S. at his residence at 1:39am, but
there was no answer. The second line indicates we then tried Bob S. on his
mobile phone within the same minute, but got no answer there either. We then
paged Bob S. to call us, and waited five minutes for a response. When Bob S.
failed to respond, we moved to calling a back-up person (John D.), and we
finally reached John on his mobile phone and relayed the message at 1:46am.
